Insurance:

I am not a participating provider with any insurance companies, and I have opted out of Medicare.

My practice is fee-for-service and payment is due at the time of each appointment. This is most often done through our portal.

Many of my patients choose to submit their own claims in order to receive partial reimbursement using their "out-of-network" insurance benefits. The receipts that I provide have all of the information that your insurance company needs in order to process your claim.

Please contact your insurance company directly to inquire about your out of network benefits prior to your first visit. If you prefer to seek treatment through an in-network provider, your insurance company can provide a list of their providers.
